This ASHP accredited PGY2 ambulatory care residency is well established and has been embedded within our health system for 20 years. Residents practice with Clinical Pharmacist Practitioners (CPP) and physicians and develop advanced skills within ambulatory care clinics to provide complex care for high-risk patients with chronic diseases including asthma, COPD, diabetes, pain, hypertension, chronic anticoagulation, etc. The resident also participates in Medicare Wellness Visits (MWV) and Transitional Care Management (TCM) follow-up telephone calls, and provides spirometry, and ankle brachial index screenings. The resident will spend 4 weeks in orientation, 6 months within a Family Medicine clinic, and 2 months within an Internal Medicine clinic, both of which are medical residency training programs. The resident will also spend at least one month as a primary preceptor for an APPE pharmacy student. Completion prepares residents for a variety of ambulatory careers. Graduates of our program and additional information can be found on our website at https://www.nhrmc.org/healthcare-professionals/residency-programs/pharmacy-residency
